Hot Spot?

I have a 5 and 1/2 month pup that got bit on the fore leg high up while running loose around the other dogs two or three months ago. He has never stopped licking it and chewing a bit. Hair is gone on the upper fore leg. One vet looked at it and said that it was not mange and said that if it continued to use Proviodine detergent on it. Well he is still licking and it is getting bigger but no open sores. Any ideas would be appreciated.
